PGH MEDICAL FOUNDATIONKung walang pambili ng gamot habang naka-admit ang pasyente sa UP-PGH, ang sumusunod ay kailangan:
1.) Letter of request mula sa doctor para kay Dr. Gregorio T. Alvior Jr. , President, PGH Medical Foundation
2.) Medical Abstract Copy
3.) Medical Prescription (reseta na may presyo – PGH Main Pharmacy, PGH Central Block Pharmacy, ADCARE Drug, Farmacia Fatima, Farmacia Wilma & K2)
Matapos makumpleto ang mga kinakailangan, ibigay sa secretary ng opisina na matatagpuan sa Nurses’ Home. Cheque ang ibibigay sa inyo na pambayad sa napiling pharmacy.
Para sa mga Laboratory / DISCHARGE BILL/ O.R. FEE/ UTZ / CT-SCAN
2.) Medical Abstract (Optional)
3.) Mga Laboratory Requests / Waiver
4.) Maikling Sulat mula sa doctor para ma-waive ang mga laboratory tests / procedures/OR Fee/ discharge bill ng pasyente
Sa pamamagitan ng MSS, ang maaaring kukunan ng pondo ay sa sumusunod: Tatak PGH, Fr. Barbero Charity Fund, Friends of PGH, Auxillary (OPD), First Gentleman, Senate Spouses Fund, PGH Alumni, Senators, etc.
Kung na-approve sa Tatak PGH, ang sumusunod ay kailangan lakarin:
1.) Siguraduhin na may Blue Card
2.) Pumunta sa MSS at magpatatak ng Tatak PGH
3.) Pumunta sa 2nd Floor sa Budget Office
4.) Pumunta sa Fiscal’s office sa 1st Floor sa Director’s office
5.) Pwede ng ayusin ang laboratory / blood request/ OR
Ang mga araw bago magTuesday, Wednesday at Friday ang pinaka-OK magsubmit ng request sa MSS
